Sample the way a reviewer samples
Build an evidence matrix rather than reading policies front to back. For each obligation, record the controlling authority, trigger, due date, person responsible, required evidence, storage location, and last internal check. Then sample several people, staff members, and events across time.
The service-recipient record requirements in section 245D.095 are a useful spine, but they are not the whole review. Staff training, program oversight, rights, incidents, health services, and setting requirements live in other sections and rules. [1][2][3]
Test practice against paper
Ask staff to explain how they find a current plan, respond to an incident, protect rights, document a change, and escalate missing information. Compare the answer with policy and the actual record. If the system requires a heroic employee to remember where everything lives, the system is the finding.
- Resolve expired, unsigned, or internally inconsistent documents.
- Verify training against job duties and first unsupervised contact.
- Trace previous correction work to implementation evidence.
- Keep a truthful list of open items with owners and dates.
Make review readiness a property of the record
nnipa is built around reviewability: source documents are read into a structured record, each generated packet carries a Need-Info view, changes are revisioned, and lifecycle dates remain visible. That reduces the scramble before a review because the same evidence used to do the work is already organized to explain it.
Questions people ask next
Should I use DHS sample policies?
They can be a starting point, but DHS states providers remain responsible for keeping policies consistent with current law and their actual program. [4]
What if I find a missing record during internal review?
Do not backdate or recreate evidence as though it existed. Document the finding, determine the required correction with qualified help, complete lawful corrective work, and preserve the trail.
