Start with a source register
Section 245D.095 requires a current service-recipient record with admission information, plans, health-service information, rights orientation, financial authorizations, incident and progress documentation, and other specified materials. The record is broader than an admission packet and must be protected against loss, tampering, and unauthorized disclosure. [1]
List every document received, its date, author, person, current version, and the fields it can support. If two sources disagree, preserve the conflict. Do not let the newest filename silently win.
Resolve the dates that drive everything else
Service initiation, admission, plan dates, assessment dates, authorization spans, and meeting dates may be different. Chapter 245D places certain actions before or upon initiation, others within 15 days, and initial assessment and planning duties before 45 days of service or 60 calendar days, whichever is shorter. A wrong anchor produces an orderly but wrong calendar. [2]
- Confirm who may consent, receive notices, and sign.
- Separate facts copied from source from provider decisions and planned actions.
- Mark absent information as absent; never invent a safe-looking default.
- Review the complete record with the responsible staff member before signatures.
Let software copy; keep people accountable
nnipa reads the source packet once, maps supported facts into the provider's forms, and carries provenance into review. It produces a pre-filled packet with a Need-Info report—not a false promise of a finished record. Through MCP, an authorized AI assistant can call the same admission and packet operations, while nnipa remains the system applying the record rules.
Questions people ask next
Is an admission packet the entire 245D record?
No. The statutory record includes ongoing plans, health information, incidents, progress, reviews, and discharge materials in addition to admission information. [1]
What should happen when two source documents disagree?
Preserve both sources, flag the conflict, and route it to the person authorized to resolve it. Do not select a value merely because it appears in a newer-looking file.
